Each year, ECRI Institute investigates hundreds of accidents, near misses, and complaints related to medical technology. Occasionally we identify product design or manufacturing defects. More often, we identify circumstances in which medical technology that is not defective is used in a manner that leads to a safety hazard. Some such use errors are one-time or rare events. Others we see repeatedly even after we have published and publicized a hazard report.
